Good Clinical Cases / Recovery Stories

A 66 year old gentleman (a pedestrian) had an accident with a two wheeler in January this year. He had open dislocation of the ankle joint with the distal part of shin bone(tibia) jutting out along with bimalleolar fracture and severe syndesmotic ligament injury. He presented to our ER within 4 hours of injury with active bleeding. We immediately planned for surgery. Debridement, wound lavage was given before reduction of joint followed by tension band wiring of medial malleolus, rush nail fixation of fibula and syndesmotic screw fixation followed by primary closure of wound. Patient luckily did not have any post operative infection or skin necrosis. He started protected weight bearing (walking) by 2 months. At 7 months all his wounds are completely healed, he is painfree with astonishingly good ankle movement. His fractures are almost united. He is very happy as he can walk pain free without any support.
Condition of wound when patient presented to our emergency room.
Pre-operative X-ray showing open ankle dislocation with bimalleolar fracture with severe syndesmotic ligament injury.
Patient was operated within 2 hours of presentation in on an emergency basis
Post op xray
Wound condition during 1st dressing- after 2 days of surgery.
At 7 months wound has healed completely
Very good ankle movements at 7 months
X-ray at 7 months showing the fractures have united well.
